What is a Prostate Digital Rectal Exam?

By: Katriena Knights

The prostate digital rectal exam is a necessary evil for men who want to assure their long-term health. While treatments for prostate cancer have become more and more effective, they still depend on early detection and diagnosis. The digital rectal exam is one of the best ways to detect the earlier stages of prostate cancer.

Why is a digital rectal exam necessary?
Because the prostate is an internal organ, the only way for a doctor to assess its condition is by touch. The prostate gland is located in front of the rectum, surrounding the urethra, so it is easily palpated through the rectal wall. The digital rectal exam is the easiest way for a doctor to feel any lumps, swelling or other irregular growth in the prostate.

How is a digital rectal exam performed?
The doctor will ask you to relax as much as possible. While this exam is generally not painful, your discomfort will be greatly reduced if you are relaxed; any tension in the anal muscles can cause the exam to be uncomfortable.

Before the internal rectal exam, the doctor will examine the exterior of the anus for fissures or hemorrhoids. Then, using a latex glove and a lubricant, he will insert his finger into the rectum and carefully palpate the prostate gland. He might ask you to take a deep breath before the exam, again to help you relax to reduce your discomfort.

The actual examination doesn't take very long, but is invaluable for your health. It can help the doctor find even small irregularities in the condition of your prostate.

How often should I have a digital rectal exam?
How often you have digital rectal exams depends on several factors. The two most important, however, are your overall health and the history of prostate cancer in your family. A general guideline is:

  • If you are African American: begin at age 40.
  • If you have family history of prostate cancer: begin at age 40 or earlier, if your doctor recommends.
  • If you are a Caucasian man with no family history of prostate cancer: begin at age 50.
  • If you experience symptoms of a prostate condition: begin immediately.

In all cases, the digital rectal exam can help your doctor diagnose any prostate condition at an early stage, when treatment for prostate cancer is most likely to be effective.
